- Voluntary Bilateral Scapulothoracic Dissociation(Servicio de Publicaciones de la Universidad de Navarra, 2015) Cruz-Morande, S. (Sebastián); Beguiristain-Gurpide, J.L. (José Luis); Duart-Clemente, J. (Julio); Llombart-Blanco, R. (Rafael)tudy design: Case report. Objective: To report a rare case of voluntary scapulothoracic dislocation and its treatment. Background data: Scapulothoracic dissociation is a rare condition. We report a case of a teenager who could volun- tarily dislocate her scapulae. Results: The patient was treated surgically by fixating her scapula to the contralateral transverse process and carrying out associated muscle plication. Now, after a 12-year follow-up, the patient is asymptomatic. Conclusions: Fixation of the scapula to the contralateral transverse process and plication of associated muscles is a valid treatment for this condition.
- Cobalt chloride allergy. Diagnosis and treatment of a rare cause of early hip prosthesis mobilization(Servicio de Publicaciones de la Universidad de Navarra, 2015) Diaz-de-Rada, P. (Pablo); Idoate, M.A. (Miguel Ángel); Muñoa, L. (Leyre); Llombart-Blanco, R. (Rafael)Purpose: Discovering the cause of early hip prosthesis mobilization is a challenge for the orthopedic surgeon. Infection is one possible cause of early prosthesis failure, but there are other causes that orthopedic surgeons should also consider. Case report: We present the case of a 61-year-old male patient with early hip failure due to cobalt allergy and describe the different diagnosis steps that led to this diagnosis. Discussion: Collaboration between departments was fundamental in order to reach the diagnosis. Revision surgery using components without the allergenic metal and using different surface bearing materials (ceramic-ceramic) gave satisfactory results.
- Perinatal outcome in women over 40 years old over a three-year period at the Clínica Universidad de Navarra(Servicio de Publicaciones de la Universidad de Navarra, 2015) Morán, M. (Mónica); Alcazar, J.L. (Juan Luis)A current social trend is for women to delay their pregnancies. Late pregnancies are commonly associated with increased perinatal pathology. We carried out a retrospective observational study to analyse the maternal and fetal morbi-mortality in pregnant women over 40 years old. Clinical data were retrieved from the medical records of all pregnant women who had their pregnancy controlled at the Clínica Universidad de Navarra (CUN) between January 2011 and December 2013. A random group of women younger than 40 years of age was used as a control group. During the study period, the CUN oversaw 1035 pregnancies, of which 102 (10.1%) concerned women over 40. Whilst the over-40 group was statistically similar to the control group with respect to most variables analysed, women over 40 had statistically higher prevalence of gestational diabetes, number of pregnancies after Assisted Reproduction Techniques (ARTs), number of previous abortions, and prevalance of high-risk results from aneuploidy.screening.
- The Ten Hallmarks of Cancer in Cutaneous Malignant Melanoma [REVIEW](Servicio de Publicaciones de la Universidad de Navarra, 2015) Broertjes, J. (Jorrit)Malignant tumors are complex and therapeutic intervention often ineffective. To bring more structure in the complex nature of tumors, the hallmarks of cancer framework was developed. This conceptual framework encompasses ten cellular functions that make normal cells malignant cancer cells. These hallmarks are: genomic instability and mutation, tumor promoting inflammation, sustaining proliferative signaling, evading growth suppressors, resisting cell death, enabling replicative immortality, inducing angiogenesis, activating invasion and metastasis, deregulating cellular energetics and finally avoiding immune destruction. These hallmarks are functional within the context of the tumor microenvironment. The hallmarks of cancer form a general framework which can be applied to interpret alterations that occur in different types of cancer. Therefore it should predict that any specific type of can- cer shows oncogenic alterations that match the hallmarks described by Hanahan and Weinberg. The type of cancer selected to test this hypothesis is melanoma. This form of skin cancer represents only 4% of human skin cancers but is responsible for 75-80% of skin cancer deaths.
